The Referral Process

The referral process at our hospital in Apopka, Florida, is designed to be seamless and straightforward. With the patient’s written authorization, our clinical team maintains active communication with the referring professional from admission through discharge. This may include sharing relevant clinical updates as treatment progresses, consulting on significant treatment decisions, incorporating the referring provider’s clinical insights into the treatment planning process and coordinating discharge planning to ensure a smooth and supported transition back to outpatient or community-based services. 

Our approach to collaboration is rooted in the belief that continuity of care is not a formality but a clinical priority. By keeping referring professionals informed and involved, we are able to deliver more cohesive, effective care and set patients up for success after they leave our hospital.
